What is Immigration Medical/Physical Exam?

The US Citizenship and Immigration Services (USCIS) requires all aliens who are applying for green cards and citizenship to complete the I-693, the Report of Medical Examination and Vaccination Record. The purpose of the medical examination is to evaluate the applicant’s health status and to determine if any conditions exist that might prevent a change in status. This examination can only be conducted by a designated Civil Surgeon. Dr. Petua Okolo will examine you, complete, certify and seal the form so that you can return it to the USCIS.

This exam requires applicants to be free of certain infectious diseases, including tuberculosis and syphilis. The HIV test is no longer mandatory. If Dr. Petua Okolo finds that you have an infectious condition that would prevent you from passing the physical examination, she can treat you or recommend a treatment plan necessary to allow you to proceed with the next step in the immigration process.

What Should I Bring to My Immigration Physical Exam?

  • A current Form i-693, pre-completed with your information. You can obtain the form from HERE. Do not date the form.
  • A valid government picture ID, such as a driver’s license or passport.
  • A copy of your vaccination records. Please note: Applicants must receive all age-appropriate vaccines as recommended by the Centers for Disease Control (CDC) to pass the physical exam.
  • Evidence of COVID-19 vaccinations is now mandatory for all Immigration applicants effective October 1, 2021.
  • An adult English-speaking translator (if you cannot speak English or Spanish).

How Much Does the Immigration Physical Exam Cost?

The lab tests required by the US Immigration under CDC guidelines for testing applicants for various diseases now vary by age group. As a result, we recently modified and lowered our fees to accommodate the changes. For basic immigration medical exam, our fees are $350 for ages 2 to 17; $435 for ages 18 to 24; $370 for ages 25 to 44; and $350 for ages 45 and above. The detail is as follows:

NOTE: The cost for any additional lab or imaging scan when required are extra (mostly for applicants with incomplete shot records or other issues):

 

  • Hepatitis B (HBsAg) Screen
  • Measles/Mumps /Rubella Immunity
  • Varicella Zoster Virus (VZV) Antibodies, IgG (Chickenpox)
  • Chest X-ray (if necessary based on test result).
  • Cost of treatment by a specialist or a County Health Department based on referral by the Physician.

Will my insurance cover the cost of the immigration medical exam?

  • No. The applicant is responsible for paying all the costs associated with the medical exam. DFW Internal Medicine Clinic does not bill insurance for immigration medical exams.
